CREDIT TERMS AGREEMENT
First transaction must be Prepaid to establish Terms. Then All Invoices are to be paid per Invoice Terms.
By submitting this application, you authorize Nampa Valley Helicopters, Inc. to make inquiries into the
banking and business trade references that you have supplied.
Accounts not paid within payment terms shall be subject to interest at the rate of 12% APR
In the event any third parties are employed to collect any outstanding monies owed by the said business
the undersigned agrees to pay NVH reasonable cost, including attorney, fees, whether litigation has
commenced, and all costs of litigation incurred. The undersigned represents that he/she has the
authority to execute this credit agreement on behalf of the business identified and acknowledges and
agrees to the terms set forth in the Nampa Valley Helicopters Component General Sales Conditions
Agreement.
